The left side of the table:
To: (Your Name)
Fax: (fax number)
Re: (Name of the open position)
Pages: (do not worry, I said, the first page)

The right side of the table (if my name is through the "To: name")
From: (Me)
Telephone: (phone)
Date: (date, duh)
